SSN, S1, polizza sanitaria — the Italian healthcare system
Overview — Italy's public healthcare system
Italy has a universal public healthcare system (Servizio Sanitario Nazionale, SSN) that covers most medical costs. The system is regionally delegated — each region has its own ASL (Azienda Sanitaria Locale) administering local services. Quality varies by region (north tends to score higher than south on most quality metrics).
For UK movers, integrating with the SSN happens through three pathways: SSN registration after qualifying residence; S1 form transferring NHS entitlement for UK state pensioners; or full private cover for those not yet eligible.
SSN registration — the standard route
After establishing residenza at your comune, you can register with the local ASL for SSN access. The registration requires the residenza certificate, codice fiscale, and proof of valid stay (permesso di soggiorno or similar). Process takes 2-6 weeks.
Once registered, you have the tessera sanitaria (health card) and access the system for medico di famiglia (GP) consultations, prescriptions, hospital care, specialist appointments at the standard reimbursement rates. The Italian SSN is generally free at point of use for most consultations and emergencies; there are modest copay (ticket) for prescriptions and some specialist visits.
S1 form — for UK state pensioners
UK state pensioners moving to Italy can transfer their NHS entitlement to Italian SSN via the S1 form, administered through the UK NHS Business Services Authority (NHSBSA) Overseas Healthcare Services. The S1 effectively means the UK government pays Italy for your healthcare.
S1 holders register at the local ASL showing the S1 form; the process is generally faster than SSN for non-pensioners. Once registered, S1 holders have the same access as Italian residents.
Eligibility: receiving UK State Pension, resident in Italy.
Polizza sanitaria — private cover
Private Italian health insurance (polizza sanitaria) covers private specialist consultations, premium hospital rooms, dental, optical, and treatments not fully covered by SSN. Many Italian residents have one as a top-up.
For UK movers, polizza is essential during the gap before SSN registration completes. Italian insurers (Generali, Allianz Italia, AXA Italia, Reale Mutua) offer expat-suitable policies; international providers (Bupa, Cigna) are also viable.
The gap period — before SSN kicks in
For UK movers without S1, there is typically a 2-6 month gap between arrival and SSN registration completing. During this period you need private cover.
Options: UK private insurer with Italian coverage; Italian polizza sanitaria; international expat health insurer. Plan €50-200 per person per month during the gap.
